C: Working with Clients and Patients face to face. 

a: You, the client understand that only through being honest and open about all matters relating to the presenting problem can you hope for a solution to the presenting problem. Without this understanding, it is unlikely you will be successful in problem resolution. 

b: I, Michael Mahoney do agree to use my knowledge and experience to help you to reach a satisfactory outcome to your presenting problem. However, no guarantees are expressed or implied. The client takes responsibility for all outcomes. 

c: The client agrees to give at least 48 hours’ notice for none attendance. Anything less may result in an invoice being raised for the total amount of the missed session.  

d: I, Michael Mahoney will do everything within my ability to provide structured sessions at a time mutually convenient to both myself and you, the client. However I, Michael Mahoney, take no responsibility for situations which fall outside of my control.  Working within an NHS facility or elsewhere outside my own premises, means I and my services are subject to the needs arising from the room provider.  Should a room not be available or circumstances change between my room provider and myself, you hold me blameless.   If such circumstance should mean the availability of a suitable room on a suitable day is not possible, I reserve the right to cancel all future sessions. Please do not engage my services if you do not agree to this clause.

e: I, Michael Mahoney will work within my knowledge and experience. I will not work in an area in which I am not / do not feel, suitably qualified. I will, however, endeavour to find a suitably qualified individual to refer you too. In this case, you are solely responsible for the engagement of any other therapist.

f: The initial consultation is an information-gathering process, no hypnotherapy session will be conducted at this first meeting. I believe having a full understanding of the clients /patient’s problem is paramount, the same goes for the client / patient understanding the therapeutic model as outlined by myself during the initial consultation. The initial consultation is chargeable at current rates.  Any and all future sessions are chargeable at current rates.

g: In the event of ill-health, accident, injury or disease occurring to myself, Michael Mahoney, or a close family member, I reserve the right to terminate all sessions if deemed necessary.  Please do not engage my services if you do not agree.

h: At times, some sessions may be recorded. this is decided by myself, Michael Mahoney, and is given free of charge, on either CD or MP3 download. These will not be available after 12 months of the session. In that time should you wish to obtain a copy of the recording, a nominal charge of £10 is chargeable.  this recording is subject to the terms and conditions set out below. User takes responsibility for all outcomes. Please do not engage my services if you do not agree. 

D: WhatsApp online sessions.

You agree to provide honest answers to the questions asked on the Initial Consultation form, in addition to agreeing to our general terms and conditions of trading.
